The study examines response bias in the assessment of breathlessness and leg discomfort during cardiopulmonary exercise testing using the modified Borg 0–10 Category-Ratio (mBorg-CR10) scale. The findings reveal a bias away from categories 6 and 8 without verbal descriptors, particularly in individuals with chronic airflow limitation. The study suggests potential limitations of the mBorg-CR10 scale and raises questions about its validity and impact on assessing disease severity and prescribing exercise intensity.
