Popis: Приведены два клинических наблюдения. У пациентов старшей возрастной группы (возраст около 90 лет) отмечалась умеренная анемия – гемоглобин около 9 г/дл. Обследование выявило нормохромную нормоцитарную анемию с признаками относительной недостаточности эндогенного эритропоэтина. Предполагалась возможность субклинически протекающего гемолиза. Была предпринята попытка назначения сверхнизких доз преднизолона (5 мг/д) или дексаметазона (0,5 мг/д) и эритропоэз-стимулирующих агентов (эритропоэтин альфа 2 тыс. МЕ 1 раз в неделю). Уровень гемоглобина у обоих пациентов несколько повысился, хотя не достиг нормальных величин. Существенным образом улучшилось самочувствие пациентов. Длительность терапии составила около двух лет. В обсуждении приводится информация, дающая материал для объяснения полученного терапевтического эффекта. Two clinical observations are given. In patients of the older age group (about 90 years), moderate anemiawasobserved– hemoglobinlevelwasabout 9 g/DL.Theexaminationrevealednormochromic normocytic anemia with the signs of relative insufficiency of endogenous erythropoietin. There was assumed the possibility of subclinical hemolysis. There was made an attempt to prescribe ultra-low doses of prednisolone (5 mg/d) or dexamethasone, and ESA (epoetin Alfa 2 thousand IU 1 time per week). The level of hemoglobin in both patients increased slightly, although it did not reach normal values. Well-being of patients significantly improved. The duration of the therapy was about two years. In the discussion, there is provided the information, which gives the material for explanation of the received therapeutic effect.
