The article discusses the use of terlipressin in hepatorenal syndrome-acute kidney injury in cirrhosis, based on recommendations from physicians in nephrology, hepatology, and critical care. The decision to remove albumin from the diagnostic criteria of HRS-AKI was made to avoid potential harm from a one-size-fits-all approach to fluid administration. The article also addresses concerns about the use of terlipressin, emphasizing the need for careful monitoring and individualized treatment. The authors recommend terlipressin as the first-line vasoconstrictor for HRS-AKI, despite limitations in existing studies comparing terlipressin and norepinephrine. [Extracted from the article] |
